ANPA 2024 Youth Conference Parental Consent/Waiver Form

The Association of Nigerian Physicians in the Americas (ANPA) will hold the 2024 ANPA Youth Conference in conjunction with the 2024 ANPA 30th Scientific Convention and Assembly, which will be held at the J.W Marriott in Anaheim, California. The Youth Conference will be held from Wednesday, June 19th, 2024, through Saturday, June 22nd, 2024.

Youth Conference Details

Parents have up to 15 minutes after the day's program to pick up their children/wards. After this, parents will be charged $10 per 15 minutes, up to a maximum of 1 hour. After this, parents will be charged $150 for one extra hour.

June 20th, 11:00 am Departure - Disneyland

June 21st, 9:30 am - 4:20 pm - Educational Activities

June 22nd, 8:30 am to 11:30 am - Bus Tour

Rules and Procedures

Age - The allowed attendee age range for participation in the youth conference will be 6 - 18 years old. Attendees will be split up into two groups. Group 1 will be ages 6 to 12, and Group 2 will be ages 13 to 18. For our Disney trip, we suggest that a parent or designated guardian for children under the age of 10 accompany their child(ren). Parents/Guardians can attend our Disney trip at the discounted cost of $250.

Chaperones - Each group will have chaperones provided by ANPA, who will oversee them on any trip or excursion. All attendees are expected to follow the lead of the chaperone and communicate with their chaperone for anything related to participation in activities and transportation.

Transportation - We will provide transportation for Disney and the Bus Tour.

Professionalism and Behavior - The attendees are expected to conduct themselves in a manner of respect and proper behavior as they will not only be representing themselves but ANPA as well.

This includes:

Adhering to the direction of the chaperone
* All participants should obtain permission from an ANPA staff member before leaving the group so that their whereabouts are accounted for (ex: bathroom breaks)

Speaking to adults, staff (ANPA staff and outside activities staff), and other conference attendees in a respectful manner

No swearing or vulgar language

Wearing appropriate attire. [T-shirts, Jeans, Casual clothing]

Conference participants should stay with the group for the duration of the program unless otherwise indicated by the parent, who should inform ANPA staff.

Youth participants should be signed in and signed out before being released.

Consequences for not following the rules of professionalism will result in immediate recourse.

PERMISSION

By signing this document, I

the parent/guardian of the attendee,

have read the content above and give the attendee permission to attend the 2024 ANPA Youth Conference, which will be held from Wednesday, June 19th, 2024, to Saturday, June 22nd, 2024, in Anaheim, California. I understand that the Association of Nigerian Physicians in the Americas (ANPA) will take necessary precautions to ensure the safety of the attendees.

I understand that it is my responsibility to ensure that my child is physically and mentally capable of participating in the activities and that my child will comply with all rules, regulations, and instructions provided by ANPA and its staff. If your child requires special services, please notify the team at volunteers@anpa.org.

I agree that the attendee's failure to follow the rules and procedures will result in consequences, including dismissal from the program.

For our Disneyland excursion, your child can participate for the whole day. You should be available to pick up your child if needed.

I agree to release ANPA from all claims and liabilities that may arise due to the attendee's actions or unforeseen circumstances that may happen to the attendee, unless due to negligence or direct cause by organizational personnel.

WAIVER

In consideration of the attendee being permitted to attend and participate in the 2024 ANPA Youth Conference conference, I

hereby agree to the following terms and conditions:

(1) Assumption of Risk: I acknowledge that while ANPA will take reasonable precautions to ensure the safety and well-being of all participants, accidents, injuries, or unforeseen incidents may occur. I agree to assume all risks associated with my child's participation in the youth activities and release ANPA from any responsibility for any injury, loss, or damage that may occur during these activities. I voluntarily assume all such risks on behalf of the attendee.

(2) Release of Liability: I hereby release, waive, and discharge ANPA, their officers, employees, agents, volunteers, and representatives from any and all claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by the actions of the attendee while attending the 2024 ANPA Youth Conference in tandem with the 2024 ANPA 30th Scientific Convention and Assembly Conference, unless they were due to to the negligence of ANPA personnel.

(3) Emergency Medical Treatment: In the event of an emergency, I authorize ANPA and its representatives to obtain medical treatment for the attendee if I am unable to be reached. I understand that the 2024 ANPA Youth Conference, in tandem with the 2024 ANPA 30th Scientific Convention and Assembly Conference, will make reasonable efforts to contact me in such circumstances.

(4) Photography and Video Release: I hereby grant permission for ANPA to take photographs and/or videos of the attendee during the 2024 Youth Conference and to use such photographs and/or videos for promotional and marketing purposes.
This waiver shall remain in effect for the duration of my child's participation in the youth activities organized by ANPA.
